Your Rights Regarding Your Protected Health Information

You have specific rights concerning your PHI, and we are committed to upholding them. You have the right to:

• Request an Amendment: If you believe any information in your medical records is incorrect or incomplete, you have the right to ask us to correct or amend it.
• Request Confidential Communications: You can ask us to communicate with you about your health information in a specific, confidential way or at a particular location. For example, you can request that we only contact you by mail at your work address.
• Ask for Restrictions: You can request a limit on the information we use or share for your treatment, billing, or healthcare operations. We are not required to agree to this request unless it involves a disclosure to a health plan for a service you have paid for in full.
• Obtain a List of Disclosures: You have the right to request a list of times we have shared your health information for up to six years prior to your request, for reasons other than treatment, payment, and healthcare operations.
• Receive a Copy of This Notice: You are entitled to a paper copy of this Notice at any time, even if you have already agreed to receive it electronically.
• Designate a Personal Representative: You can appoint a person to act on your behalf regarding your health information. You must provide us with written authorization for this designation.
• File a Complaint: If you believe your privacy rights have been violated, you can file a complaint directly with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Your Choices About How We Use and Share Your Information
